After sending in this form you will receive an invoice calculated on the chosen registration packages. You can pick a different pack for each puppy.

After the payment is received by ISA you will be sent the swabs for each DNA test as well as a form to register the names and chipnumbers of each puppy. If you want to restrict any of the registrations other than the restricted package this can be chosen on the form.

Registration of individual puppy information can be done up to 5 months of age.
